Patient Satisfaction

Your comfort and concerns are very important to us and we want to know how to better serve you. If you  have questions or comments about the services you have received or suggestions for ways to improve our delivery of care, please consider the following:

  1. Contact your nurse or director/clinical manager of your patient care area to address the issues or concerns. Every effort will be made to resolve those issues while you are still a patient here.
  2. There may be concerns which require more extensive follow-up or which could not be answered to your satisfaction. To request a copy of our “Report of Concern” form during your stay, please contact your nurse or director/clinical manager of your patient care area. The form will be brought to you for completion and will be returned to the appropriate department director/clinical manager for follow-up action.
  3. Randomly selected patients may receive a patient satisfaction survey, which is mailed to their home shortly after discharge. This survey provides an opportunity for you to let us know your thoughts about your care during your stay. We cannot improve our services without your input and we appreciate your comments. Any problems or concerns identified in the survey are directed to the appropriate department director/clinical manager.
  4. If you would rather talk to someone personally to share your concerns, either during your hospital stay or after you leave, please call our Patient Services Department at 920-361-5500 or toll-free at 1-800-236-1283, extension 5500.
